At the appointment you will be asked to provide family income information, proof of residency and identity, and information about past and current health. You will also be required to measure your height and weight, take a finger stick blood test and visit with a health professional about nutrition education and health needs. Infants up to 9 months of age will not be required to take a blood test. If eligible, you will get food “checks” to buy foods at authorized grocery stores.

WIC is an equal opportunity provider. More information about the program is available on the Department of Health web site at

http://doh.sd.gov/family/wic/.
